adults driving students-including to and from practice) - Form G8: Parent/Guardian Permission Form for a Student to be a Passenger in a Private or Rental Vehicle (Required if swimmer will be in some else's care-including to and from practice)
- Form G8: Parent/Guardian Permission for a Student to Operate a Private Vehicle for School Activities (Required if your swimmer is giving rides to others including to and from practice)
